If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill. You will need to complete a New Patient Questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Medical treatment is available from the date of registration. Please contact reception for further information.

If you have your medical card, please bring it with you to register at the practice. However, if you do not have your medical card, you can complete an HS200 Form, for registering with the practice, or an HSCR1 Form depending on your circumstances. When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. If you wish to complete this in advance, please click on the link above to open the relevant form, to print and complete at your convenience.

Please bring the completed forms with you along with photographic ID and proof of residency (utility bill etc), to the reception desk, when you are registering.

Temporary Registration: for UK/EU Citizens

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice. You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply call in to reception, or complete the ‘Temporary Residents form’ above. You cannot register as a temporary patient at a practice in the town or area where you are already registered, of if your own GP surgery is within a reasonable travelling distance.
