IMPACTAgewell® is a new service offered through Meadowbridge Surgery which involves an IMPACTAgewell® Officer who will call out to your house for a few visits to have conversations about your health and wellbeing and discuss any additional services or support that might be available to help you, in addition to those currently offered by the GP practice.

This could include for example, linking you to local groups or agencies to help you find out about what’s on in your local community, transport services available, advice on the most efficient way to heat your house and how to ensure your home is safe and secure.

The IMPACTAgewell® Officer can also link in with the local Northern Health & Social Care Trust should you need to be assessed for aids and devices to help you stay independent in your house, and also link in with the services offered by your local Community Pharmacy to assist you in managing your medications.

So if you are agreeable, we would like to gain your consent to pass you on to Deborah or Andrea, our IMPACTAgewell® Officer, who will contact you and arrange your first home visit and give you a chance to ask any questions you may have about the programme.
